Fear Effect combines hard-edged gunplay, occult horror, and a pulpy crime-story attitude, with a deliberately adult tone that feels closer to a violent late-night thriller than a conventional heroic adventure. Its cel-shaded characters move through animated video backdrops, giving every alley, casino, and hellish location an unusual, surreal energy.

Play shifts between the three protagonists as they investigate environments, collect key items, solve inventory-based puzzles, and fight armed thugs or monstrous enemies. Combat uses an aiming system that rewards careful positioning and ammunition management, while the trademark Fear meter rises when characters take damage or face danger, leaving them more vulnerable when panic sets in. Fixed camera angles and old-school controls make movement feel rooted in the original PlayStation era, so encounters can be demanding, especially when enemies attack from awkward directions. Boss fights, branching conversations, environmental interaction, and occasional consequences for player decisions keep the roughly seven-to-nine-hour campaign moving beyond simple corridor shooting.
